How to Read Clinical Notes and Write Notes Back to the EHR with
Blog post from Unified.to
Published October 24, 2026, the guidance describes Unified.to’s Healthcare API for reading clinical documents from eleven EHR platforms through a single connection and standardized Document object, including progress notes, consultation notes, discharge summaries, and other narrative records. The API normalizes note types and metadata such as patient, encounter, author, status, timestamps, and original coding, while providing short-lived signed URLs to retrieve PDF, HTML, or text content directly from the customer’s EHR without exposing EHR credentials or storing patient records at rest. Applications can list and filter documents, account for amended or erroneous notes, and join notes to encounter and practitioner data. TruBridge additionally supports creating and filing base64-encoded clinical notes, while other integrations are currently read-only and require capability checks. Support varies by EHR, with unsupported document access returning a 501 response rather than an empty list, and selected platforms offering virtual webhooks and timestamp-based updates to detect newly created or amended documents.
