Summary

A 2026 theCUBE Research and Qualitate study of 25 enterprise technology and security leaders found that while 19 organizations had AI agents in production, only four had tested an AI or agent incident scenario, highlighting a preparedness gap for failures that may occur without outages, compromised credentials, or obvious policy violations. Respondents commonly relied on human oversight, with none permitting fully unsupervised high-risk actions; however, the study suggests this approach may become difficult as agent activity scales. Key weaknesses included detecting anomalous behavior, attributing actions to agents rather than users, reconstructing cross-system activity, remediating changes, and identifying a last known trusted business-process state. The report argues that shutdown mechanisms can stop further actions but cannot reverse consequences such as sent communications or external disclosures, and recommends exercising realistic scenarios in which authorized agents make harmful decisions across functioning systems. TestMu AI positions its Rook staging tool as a complementary pre-release control that tests an agent’s actual changes and tool use, while noting that it does not provide runtime blocking, containment, rollback, or recovery-plan testing.
