The Hidden Reason Most RAG Projects Never Reach Production
Blog post from FalkorDB
Enterprise RAG deployments frequently fail to reach production because their retrieval layers are not designed for continuously changing, complex organizational data, according to a cited 2026 analysis. The text identifies stale vector indexes, weak entity resolution, lack of answer traceability, and poor handling of relationship-heavy, multi-hop questions as central challenges for vector-based RAG systems. It argues that knowledge graphs can address these issues through incremental updates, canonical entity nodes, explicit and auditable traversal paths, graph-native access controls, and relationship traversal. FalkorDB’s GraphRAG SDK is presented as an implementation of this approach, with a reported 71.48 score on GraphRAG-Bench versus 55.39 for a reranked vector-RAG baseline using the same generation model. It recommends diagnosing incorrect answers by determining whether the underlying issue is absent provenance, faulty retrieval, or, less commonly, model reasoning.
