CIO confession 3 of 6: Your next AI incident could start with a green dashboard
Blog post from Dataiku
Dataiku’s 2026 CIO survey of 685 enterprise leaders finds that AI agents can create business risk even when they function correctly according to technical metrics: 79% of CIOs report agents acting against business intent, policy, or expectations while remaining within their programmed parameters. As agents increasingly execute workflows and decisions at scale, with two-thirds of CIOs estimating that at least 51 are in production, conventional monitoring of uptime, speed, and error rates is insufficient to detect drift caused by changing prices, policies, customer circumstances, or organizational priorities. Nearly one-third of respondents said agent incidents had already caused customer, financial, compliance, or operational harm, and most organizations cannot contain a problematic agent within the same day, allowing machine-speed errors to compound across systems. The report argues that effective agent management requires three linked capabilities: visibility into agent decisions and actions, continuous assessment of behavior against defined business intent and expected outcomes, and rapid intervention tools to pause, modify, or retire agents when needed.
