Why the safest AI teams report the most security incidents

Low AI incident counts may reflect weak monitoring rather than strong security, particularly as organizations expand the use of AI tools and agents. A Box and Harris Poll survey of 1,640 IT decision-makers found that nearly half had experienced an AI-related information exposure, while more mature AI adopters reported incidents more frequently, likely because they operate more AI systems and have better detection capabilities. Many less mature organizations have not audited for AI incidents or lack visibility into employees’ AI use with company data, making clean reports unreliable indicators of safety. Effective AI governance requires both visibility and controls, beginning with standards for agent access to data and extending to measurement of actual exposure and detection capability rather than only operational performance. Leaders are encouraged to ask whether their organizations would detect an incident, rather than simply how many occurred, because stronger visibility can increase reported incidents while enabling safer, faster AI deployment.
